Analysis of the therapeutic effects of platinum based drugs for advanced breast cancer with different number of treatment lines and different molecular types

Abstract:Objective To explore the differences in the therapeutic efficacy of platinum based drugs for advanced breast cancer with different number of treatment lines and different molecular types.Methods A retrospective analysis of 270 cases from Affiliated Tumor Hospital of Xinjiang Medical University with advanced breast cancer,who treated with platinum based drugs from January 2010 to January 2015 in hospital were included.The differences in the therapeutic efficacy,prognosis and safety of different types of treatment lines and molecular types of advanced breast cancer were compared.Results The DCR in patients with first line and second line therapy were significantly higher than those in patients with third line and above (83.87%,72.92% and 54.46%,x2 =17.063,P<0.001);the DCR in patients with ER(+) (including PR(+/-) was significantly higher than that in patients with ER(-) (86.21%,74.24% and 60.23%,x2 =17.021,P <0.001);the DCR of HER2 (+) patients was significantly higher than that of HER2 (-) patients (76.54% and 64.02%,x2 =4.071,P =0.044);the DCR of TNBC patients was significantly higher than that of non TNBC patients (71.71% and 55.38%,x2 =6.021,P =0.014);the DCR in patients with carboplatin and lobaplatin were significantly higher than those in patients with cisplatin (67.59%,77.78% and 55.56%,x2 =9.047,P=0.011).The median TTP was 9 (4-14) months for all patients;TTP in first line patients was significantly higher than that in third line and above patients (x2 =14.087,P <0.001);The TTP of ER(+)/PR(+) patients was significantly higher than that of ER(+)/PR(-) and ER (-) patients (x2 =35.935,P<0.001);There was no significant difference in TTP between HER2 (+) and HER2 (-) patients (x2 =2.707,P =0.100);The TTP of TNBC patients was significantly higher than that of non TNBC patients (x2 =24.636,P <0.001),the TTP in patients with carboplatin and lobaplatin were significantly higher than those in patients with cisplatin (x2 =12.023,P =0.002).There were 71 cases with grade 3 or more adverse reactions;the incidence rate was 26.30%.The incidence of lobaplatin was significantly lower than that of cisplatin (18.89% and 36.11%,x2 =6.085,P =0.014).Conclusion The efficacy and prognosis of platinum based chemotherapy in patients with first line treatment,TNBC and ER,PR,and HER2 (+) positive are better for advanced breast cancer.The efficacy and safety of lobaplatin is better than that of cisplatin.
