Effects of melatonin on oxidative stress and inflammatory state in patients with chronic renal failure undergoing ma-intenance hemodialysis

Abstract:Objective To observe the effects of melatonin on oxidative stress and inflammatory state in patients with chronic renal failure undergoing maintenance hemodialysis.Methods One hundred and twelve patients with chronic renal failure were randomly divided into the control group and the observation group,with 56 cases in each group. The control group was treated with routine hemodialysis and anemia improvement. The observation group was treated with melatonin on the basis of the treatment of the control group. Serum levels of tumor necrosis factor alpha (TNF-α), interleukin 1 beta (IL-1 beta) and interleukin 6 (IL-6) were observed before and after treatment in two groups to evaluate the level of inflammation. At the same time, malondialdehyde (MDA), hydroxyl superoxide dismutase (SOD), glutathione peroxidase (GSHPx) and ad-vanced oxidation protein products (AOPP) and other indicators were observed to evaluate the level of oxidative stress in the patients. The incidence of adverse events was also observed during treatment.Results The 2 groups after treatment, TNF-α alpha and IL-1 beta and IL-6 level decreased (t/P in control group:2.201/0.030,7.717/ <0.001,8.578/ <0.001; t/P in observation group:3.120/0.002,1.984/0.048,1.975/0.037),and there was significant difference between the control group and the observation group (t/P between the two groups after treatment:2.290/0.024、2.115/0.037、2.665/0.009). The difference of MDA,SOD,GSHPx and the AOPP index of 2 groups were not statistically significant (P>0.05); the 2 groups after treatment, MDA and AOPP levels were significantly higher than those before treatment, SOD and GSHPx de-creased in the control group (t/P in control group:7.292/0.028,6.299/0.025,5.518/0.033,5.017/0.035; t/P in ob-servation group:4.419/0.039,3.118/0.042,2.285/0.039,3.308/0.047),and there was significant difference between the 2 groups in the above indicators (t/P between the two groups after treatment: 8.443/0.027, 11.271/0.014, 4.419/0.023,7.228/0.020). Adverse reactions in 2 groups were no significant differences (χ2=0.259,P=0.611).Conclusion Melatonin can effectively alleviate oxidative stress and inflammation in patients with chronic renal failure undergoing mainte-nance hemodialysis,and the safety is good.
