Application of scoring system for diabetic foot combined with coronary heart disease in clinical practice

Abstract：Objective To investigate the effect of coronary heart disease scoring system in diabetic foot patients with coronary heart disease.Methods Two hundred and ninety-seven cases of diabetic foot patients from November 2013 to November 2014 in Beijing General Hospital of the Air Force PLA were enrolled as the research object,according to whether the patients with coronary heart disease,they were divided into 2 groups:CAD group and control group.The risk factors of diabetic foot screening for patients with coronary heart disease,the factors of multiple values,establish coronary scoring system,the effects of the application of scoring system were analyzed.Results Compared with the control group,CAD group gender (x2 =5.539,P =0.036),age (t =8.978,P =0.002),systolic blood pressure (t =4.125,P =0.039),diastolic blood pressure (t =3.003,P =0.045),body mass index (x2 =4.786,P =0.036),triacylglycerol (t =2.452,P =0.046) were statistically different (P ＜0.05);diabetes duration (t =10.325,P =0.001),blood uric acid (t =7.569,P =0.006),left ventricular ejection fraction (t =8.978,P =0.002) with significant difference (P ＜ 0.01).DF combined with multivariate Logistic regression analysis indicated that age of coronary heart disease (OR =1.122,95％ CI 1.011-1.229,P =0.028),diabetes (OR =0.189,95％ CI 1.125-1.245,P =0.001),left ventricular ejection fraction(OR =0.211,95％ CI 1.061-1.278,P =0.004),body mass index (OR =0.325,95％CI 1.102-1.231,P =0.036),blood uric acid (OR =0.121,95％CI 1.009-1.228,P =0.027) were independent risk factors for coronary heart disease in diabetic foot.The risk factors of coronary heart disease with DF according to the risk factors associated with coronary heart disease in diabetic foot were assigned to establish the scoring system,the score system of area under the ROC curve was 0.758 (0.682-0.835),the standard error is 0.039,which has moderate effectiveness.Conclusion Coronary heart disease scoring system has a good predictive value for diabetic foot and coronary heart disease,and can provide a basis for diagnosis of diabetic foot patients with coronary heart disease.
