The effect of different dose of metoprolol on cor pulmonale and on patients with cardiac function improvement and adverse events

Abstract:Objective To study and analyze the effect of different dose of metoprolol in patients with cor pulmonale and cardiac function improvement,prevention of adverse events,provided a reference for choosing appropriate drug dosage for clinical treatment.Methods Two hundred and forty-six patients with pulmonary heart disease admitted to the respiratory department of Hanyang Hospital of Wuhan University of Science and Technology from June 2012 to March 2016 were selected as the subjects.They were randomly divided into the target dose group (first dose of 6.5 mg/d,double after every 14 d,until the target dose) and fixed dose groups (25 mg/d),and the rest were the same treatment,compare the clinical effect after 180d.Results Target dose group had better clinical curative effect,patients with cardiac function was significantly improved,the treatment the total effective rate was 95.0%;Fixed dose treatment group total effective rate was 79.7% (P < 0.05).Compared with pre operation,the 2 groups after treatment,RVSP decreased,PASP increased,LEVF index increased,HR and BP decreased,the observation group changed more significantly (t =19.816,t =3.759,t =3.892,t =3.659,t =7.620;P <0.001,P =0.016,P =0.038,P =0.037,P < 0.001).Two groups all have dizziness,bradycardia,weight gain and other adverse events occur,but the target dose group's total incidence was 8.0%,better than the 4.8% of fixed dose groups (x2 =2.335,P > 0.05).Conclusion Target dose of metoprolol apply to patients with cor pulmonale had better clinical curative effect,can effectively improve the heart function and does not increase the risk of adverse events.
