The diagnostic value analysis of Cyst C, lipoprotein a, mAlb combined detection in diabetic nephropathy

Abstract：Objective To investigate the serum the diagnostic value analysis of cystatin C(CysC),lipoprotein(a)(LP-a),micro albumin analysis(mAlb)for chronic kidney disease(CKD).Methods 124 patients with type 2 diabetes mellitus(DM)were randomly divided into normal albuminuria(NA,n=35)group,microalbuminuria(MA,n=45),clinical white proteinuria(CP,n=44)group,healthy control group(n=35)according 24h urinary albumin excretion rate(UAER).The levels of CysC,LP-a were measured with ELISA,urinary mAlb was measured with biochemical indicators.Results The levels of BMI,WC,FPG,2 hPG,HbAlc,FC-P,2 hC-P,insulin resistance index(HOMA-IR),serum creatinine(SCr),blood urea nitrogen(BUN),CysC,LP-a,mAlb of NA group,MA group,CP group were significantly higher than control groups(P<0.05).The levels of BMI,WC,FPG,2 hPG,HbAlc,CP,2 hC-P,HOMA-IR,SCr,BUN,CysC,LP-a,mAlb of CP group was higher than NA group,MA group.The LP-a was positively correlated with UAER,BUN by spearman univariate analysis(r=0.318,0.330,P=0.010,0.000),CysC was positively correlated with UAER,BNU,SCr(r=0.326,0.308,0.312,P=0.000,0.010和0.008),urinary mAlb was positive correlation with HbAlc(r=0.342,P=0.000).By subject-specific curve(ROC)analysis,CysC,LP-a,when mAlb joint detection sensitivity(91.26％),high specificity(85.63％).Conclusion Serum CysC,LP-a,urinary mAlb level and is closely related to the occurrence of CKD,serum CysC,LP-a,urinary mAlb level can be used as an early diagnostic marker CKD.
