Clinical value of fiberoptic bronchoscopy adjuvant therapy in ICU patients with ventilator associated pneumonia

Abstract：Objective To analyze and investigate the clinical value of fiberoptic bronchoscopy in patients with ventilator associated pneumonia.Methods Ninety-six cases of ICU patients with ventilator associated pneumonia were selected,who were treated in hospital from May 2013 to May 2016,and they were divided into the observation group(48 cases)and control group(48 cases).The patients of two groups were all treated with anti-infective therapy.The patients of observation group were treated with fiberoptic bronchoscopy lavage,while the patients of control group were treated with routine lavage and aspiration.The therapeutic effect,the clinical index and the related respiratory mechanics parameters of two groups were observed and compared.Results After the appropriate treatment,the patients of the observation group got a total effective rate of 95.8％,which was significantly better than the 83.3％of the control group(x2=9.442,P=0.009);The fever time,mechanical ventilation time,white blood cell count and CPIS score of the observation group were significantly lower than those of the control group(t=18.066,23.017,8.295,14.448,P<0.01);In the terms of PIP,Cdyn,Raw and WOB,the patients of two groups were all significantly improved than before(P<0.01),and the patients of the observation group got more significant improvement(t=9.974,14.237,15.449,4.388,P<0.05);In the terms of CRP,IL-8 and PCT,the patients of two groups were all significantly decreased than before(P<0.01),and the patients of the observation group got more significant decline(t=24.077,13.548,12.642,P<0.05).Conclusion In adjuvant therapy of ICU patients with ventilator associated pneumonia,fiberoptic bronchoscopy can obtain some good clinical results,promote the infection control in patients,reduce the inflammatory factors levels,and can effectively improve the patient''s respiratory function,which makes it an important clinical application value.
