Effect of sequential rehabilitation in treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure and the effect to pulmonary surfactant protein D, Clara cell secretory protein level

Abstract:Objective To explore the effect of sequential rehabilitation in treatment of chronic obstructive pulmonary disease with type II respiratory failure and the effect to pulmonary surfactant protein D, Clara cell secretory protein.Methods Ninety-two patients with chronic obstructive pulmonary disease complicated with type II respiratory failure were treated from February 2015 to July 2016 in our hospital were selected, they were divided into the control group (46 cases) and the observation group (46 cases) according to the random number table method, the control group was given routine treatment, the observation group were given sequential rehabilitation on the basis of routine treatment.The clinical efficacy after 3 months of the two groups were observed, simple mental state scale (MMSE) evaluation of cognitive function, Bathel index to evaluate the ability of daily life, pulmonary function index of lung function instrument, ELISA method for determination of serum surfactant protein D (SP-D), Clara cell secretory protein (CC16) and TNF-α, IL-8, 6min walking distance, dyspnea and quality of life were evaluated.Results The total effective rate of the observation group was 80.43%, which was higher than that 95.65% of the control group(χ2=5.059,P=0.024).The MMSE score and Bathel index in the observation group were higher than those in the control group (t=6.474,t=5.626,all P=0.000).The MVV, PEF, FEV1 and FVC1 of lung function indexes in the observation group were higher than those of the control group (t=8.110,t=5.213,t=5.329,t=7.442,all P=0.000).The SP-D, CC16, TNF-α and IL-8 in the observation group were all lower than those in the control group (t=1.684,t=18.224,t=20.252,t=7.998,P=0.048,P=0.000,P=0.000,P=0.000).6 min walking distance in the observation group was higher than that in the control group, and the dyspnea and quality of life were lower than those of the control group (t=10.057,t=4.261,t=4.153,all P<0.01).Conclusion Sequential rehabilitation therapy for chronic obstructive pulmonary disease with type II respiratory failure has a good clinical effect, can improve the cognitive function and life ability, enhance pulmonary function, reduce the level of SP D, CC16 and inflammatory factors, ease breathing difficulties, improve the quality of life.
