Application of end tidal carbon dioxide tension monitoring acute exacerbation of chronic obstructive pulmonary disease patients

Abstract:Objective To study the correlation between end tidal carbon dioxide partial pressure and arterial carbon dioxide partial pressure in patients with acute exacerbation of chronic obstructive pulmonary disease,and to evaluate its clinical significance.Methods A total of 40 patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and 29 normal adults took pulmonary function tests,including FEV1,FEV1/FVC,MMEF,RV/TLC and sRAW.The values of end tidal carbon dioxide tension (PetCO2) and arterial carbon dioxide tension (PaCO2) were measured at the same time,and then the value of ADCO2 was calculated.Results (1) The FEV1,FEV1/FVC and MMEF values of AECOPD patients were significantly decreased (t =4.52,t =4.22,t =3.80,P =0.039,P =0.027,P =0.021),and the values of sRAW and RV/TLC were significantly increased (t =6.31,t =1.73,P =0.041,P =0.0087).(2) Compared with normal control group,the value of PaCO2 in AECOPD group increased significantly (t =2.11,P =0.034).The value of PetCO2 in AECOPD group were significantly lower than this in normal control group (t =1.83,P =0.029).PetCO2 in both of the groups had significant correlation with PaCO2 (r =0.96,r =0.63,all P <0.01).ADCO2 in AECOPD group was significantly associated with FEV1,MMEF,SRAW and RV/TLC (P < 0.05).Conclusion The PetCO2 and PaCO2 have a good correlation,PetCO2 can be used to judge the change of PaCO2 in normal and AECOPD patients,and the status of respiratory function in patients with AECOPD.ADCO2 reflects the obstruction degree of ventilation,and may be used to evaluate the severity of AECOPD.
