High risk factors of cardiac valve calcification in hemodialysis and peritoneal dialysis patients and its effect on clinical prognosis

Abstract：Objective To analyze the risk factors of cardiac valve calcification in hemodialysis and peritoneal dialysis patients, and to analyze the effect of cardiac valve calcification on the clinical outcome.Methods From January 2014 to June 2014, 67 patients in department of renal medicine in Jiangmen Central Hospital on MHD and 63 patients on CAPD in hospital for more than 3 months were selected.Compared the clinical data between two groups of patients; Logistic regression model was used to analyze the risk factors for valve calcification.Results Hemodialysis group valvular calcification incidence was higher than that of PD group (64.18% vs.46.03% χ2 =4.327, P =0.038).2 groups of patients with age, valvular calcifi-cation cases, dialysis duration, systolic blood pressure, residual renal function, serum phosphorus, calcium phosphorus prod-uct, Alb, LDL-C, HDL-C, the difference was statistically significant ( t =2.025,χ2 =4.327, t =3.652, t =2.191, t =4.091, t =3.205, t =2.378, t =2.117, t =1.990, t =2.314, P <0.05).Logistic regression analysis showed that older age, longer dialysis time, hyperphosphatemia and residual renal dysfunction were independent risk factors of cardiac valve cal-cification (OR =1.023, OR =1.261,OR =0.436, 95%CI were 1.023 -1.359, 1.011 -1.186,0.221 -0.846).Follow up survey showed that 16 cases of death, including cardiac death in 12 cases, 4 cases of non-cardiac death.Valve calcifica-tion was significantly predictive of cardiac death and non-cardiac death ( P <0.05).Conclusion Age, duration of dialysis, diabetes mellitus, residual renal function and high phosphorus were all independent risk factors leading to the occurrence of cardiac valve calcification.The residual renal function of peritoneal dialysis patients was higher than that of hemodialysis pa-tients.The incidence of blood phosphorus and valve calcification was lower than that of hemodialysis patients.
