Effect of preoperative hepatic artery combined with regional arterial infusion chemotherapy on the prognosis of pa-tients with colorectal cancer

Abstract:Objective To observe the effect of preoperative hepatic artery combined with regional arterial infusion chemotherapy ( HRAIC) on the recurrence and metastasis of cancer cells and the survival time of patients with stage III colo-rectal cancer.Methods One hundred and fifty-seven admitted stage Ⅲ colorectal cancer patients of interventional radiology department of cancer hospital of China Medical University from January 2010 to December 2012 were selected as study objects and they were randomized into study group (77 cases) and control group (80 cases) , the study group applied with preopera-tive HRAIC neoadjuvant chemotherapy 7 d before radical resection, and with FOLFOX4 6 chemotherapy postoperatively, the control group without preoperative neoadjuvant chemotherapy, and the rest was the same as study group;follow up after treat-ment, and tumor markers after the treatment, postoperative metastasis and recurrence time, metastases& recurrence rate and survival rate were compared.Results The levels of CEA, CA19-9 at the end of 6 months in the study group after treatment were lower than those in control group [(13.4 ±5.5) μg/L vs.(20.7 ±7.2) μg/L, (30.5 ±9.5) U/ml vs.(37.2 ± 12.1) U/ml, t =7.155, P <0.01;t =3.867, P <0.01].The study group during the follow-up period, tumor metastasis or recurrence time was longer than that of the control group [(29.1 ±6.5) month vs.(24.4 ±6.1) month, t =2.169, P =0.041, liver metastasis rate was lower than the control group (9.1% vs.21.3%,11.7% vs.23.8%, χ2 =4.479、3.895, P =0.034).The end of the study group after treatment for 3 years, 4 years survival rate was higher than the control group (87.0%vs.78.8%, 83.1%vs.73.8%,χ2 =1.881,χ2 =2.029, P =0.170, P =0.154);the study group cumulative di sease-free survival probability and the cumulative probability of survival was significantly higher than the control group(86.5%vs.64 a.1%,82.4%vs.66.3%,χ2 =159.130,84.336, P <0.01).Conclusion Preoperative HRAIC treatment can signifi-cantly reduce the incidence of postoperative tumor load and liver metastasis, prolong the time of metastasis and recurrence, and improve the survival rate after radical resection of colorectal cancer.
