The relationship of the levels of cortisol, adiponectin,nitric oxide and prognosis in elderly patients with acute trauma

Abstract:Objective To investigate the levels of serum cortisol ( COR) , adiponectin ( APN) , nitric oxide ( NO) , interleukin 6 ( IL-6) , tumor necrosis factor-α( TNF-α) in elderly patients with acute trauma.Methods From 2015 January to 2015 December, 45 cases of the elderly patients with acute trauma ( including 25 cases of low risk subgroup, 20 cases of high-risk subgroups) treated in the emergency medical department of Hanzhong Central Hospital of Shanxi province were en-rolled, another 40 cases of normal physical examination in our hospital were selected as healthy control group.The levels of cortisol, APN, NO, IL-6 and TNF-αwere determined by ELISA method, the changes of 1 d, 3 d, 5 d,7d, APN, TNF-α, NO, IL-6 and in trauma group were observed.Application of acute physiology and chronic health evaluation system II ( A-PACHE II) to assess the severity of the disease, and the relationship between serum cortisol, APN, NO, IL-6, TNF-αand A-PACHE II was analyzed by using Pearson correlation analysis.Results The levels of serum cortisol, NO, IL-6 and TNF-αin trauma group were significantly higher than those in healthy control group, while APN level was lower than that in healthy con-trol group, the difference was statistically significant ( P <0.05).Compared with the preoperative, after surgery, patients'1 day, 3 days, 5 days, 7 days blood cortisol, nitric oxide ( NO) , IL-6 and TNF-αlevels were significantly decreased ( F =15.963, F =10.452, F =20.125, F =12.963, P <0.05), and APN level increased significantly ( F =17.222, P <0.05).Compared with the low risk group, the levels of serum cortisol, NO, IL-6 and TNF-αwere higher in the high risk sub-group, while the APN level was decreased ( t =15.356, t =13.287, t =9.256, t =32.088, t =-8.879, P <0.05). Pearson correlation analysis showed that serum cortisol, NO, IL-6, TNF-αand APACHE II were positively correlated ( r =0.326, r =0.302, r =0.348, r =0.310, P <0.05), while APN and APACHE II was negatively correlated ( r =-0.339, P <0.05).Conclusion Dynamic monitoring in elderly patients with acute traumatic patient's blood cortisol, APN, no and IL-6 and TNF-αlevels will help to timely and accurately make judgment and predict the prognosis of patients, which is helpful in the clinic in a timely manner to develop an appropriate treatment program.
