Comparison of subtotal colectomy with cecorectal anastomosis and ileosigmoidal anastomosis for the treatment of slow transit constipation

Abstract:Objective The aim of the study was to compare the efficacy of ileosigmoidal anastomosis ( ISA ) with cecorectal anastomosis ( CRA ) after subtotal colectomy in patients with slow-transit constipation.Methods From January 2006 to December 2013, 62 patients who were confirmed slow transit constipation in Research Center of Coloproctology of He-bei Province were enrolled, according to randomly number table, they were divided into ISA and CRA group with 31 cases each group.The operation time, bleeding volume during operation, postoperative hospital stay, use of postoperative antidiar-rheal agents, incision infection, anastomotic leakage, frequency of defecation a week, abdominal bloating score, abdominal pain score, properties of defecation, incidence of complications, operation satisfaction score, gastrointestinal quality of life in-dex ( GIQLI) in both groups were compared.Results Postoperative defecation frequency, abdominal pain score, abdominal bloating score both in two groups were improved significantly compared with preoperative data ( P <0.05).Compared with the CRA group, defecation frequency,abdominal bloating score and postoperative complications in ISA group were lower than those in CRA group,there were significant differences( t =2.165,2.021,11.273, P <0.05);but properties of defecation, operation satisfaction score and GIQLI in ISA group were significantly better than those in CRA group ( t =6.513,3.663, 5.310, P <0.05) .Conclusion Ileosigmoidal anastomosis is superior to cecorectal anastomosis for slow transit constipa-tion, patients can get better quality of life and satisfaction besides relieving the constipation.
