Effect of low molecular weight heparin in cancer patients with hypercoagulable states and survival analysis

Abstract:Objective To explore the effect of low molecular weight heparin in malignant tumor patients with hyperco-agulable state and survival rate.Methods From May 2010 to May 2013, 300 cases of malignant tumor patients who were treated in the hospital were selected, clinical data and blood coagulation state, analysis of risk factors of high coagulation state were observed and recorded, and 90 patients with hypercoagulable state and D-dimer body ( DD) positive and no thrombosis were screened, they were divided into the observation group and the control group according to random number table method, 45 cases in each group.The control group received conventional symptomatic treatment and chemotherapy, the observation group was given low molecular heparin 5000 U with the abdomen subcutaneous injection, continuous used for 4 weeks.Before and after treatment, the DD level, coagulation index [ fibrinogen ( Fib) , activated partial thromboplastin time ( APTT) ] chan-ges, thrombosis rate, survival rate and adverse reaction in the two groups were observed.Results After treatment, observa-tion group's DD and Fib levels were significantly lower compared with before treatment ( P <0.01) and APTT time did not show significant changes ( P >0.05);in the control group, before and after treatment, these indices did not show significant differences ( P >0.05), after the treatment, observation group's DD and Fib levels were lower than the control group ( P <0.01);lymph node metastasis, TNM staging and histologic types were correlated with high coagulation state, which maybe the risk factors ( P <0.01).The incidence of thrombosis in observation group was lower than that in control group (4.4%vs. 26.7%), and the survival rate was significantly higher than that in control group (88.89 vs.75.56%), the differences were statistically significant ( P <0.05).45 cases of malignant tumor patients with low molecular heparin treatment, 2 cases of pa-tients with gingival bleeding and the amount is small, which did not affect the treatment.Conclusion It demonstrated that the adenocarcinoma, combined with lymph node metastasis, and the advanced cancer patients are more prone to have blood coagulation, low molecular heparin can significantly reduce the blood coagulation state, reduce the incidence of VTE, improve the overall survival rate, it is worthy of clinical application.
