The influence of Sitagliptin on serum levels of pro inflammatory and anti inflammatory cytokines in patients with au-toimmune diabetes

Abstract:Objective To observe the effect of sitagliptin on the level of inflammatory cytokines and anti inflammatory cytokines in patients with autoimmune diabetes mellitus ( LADA) .Methods Selected 36 cases of LADA patients from Janu-ary 2012 to December 2014 treated at Hubei Zhongxiang City People 's Hospital, they were randomly divided to sitagliptin (n =18) group and insulin group ( n =18), and 20 cases non-diabetic healthy volunteers with gender, age-matched as healthy control group.After treatment of 3 months, inflammatory factor (INF-γand TNF-α, IL-1βand IL-6) and anti-inflam-matory cytokines (IL-10, TGF-β, IL-4) index were compared.Results Before treatment, LADA serum proinflammatory cy-tokines were significantly higher than control group ( P <0.05).After treatment, sitagliptin group patient's TNF-α, IL-1βand IL-2 level decreased most obviously ( P <0.05), and insulin group only IL-6 levels decreased with statistical significance ( P <0.05), and sitagliptin group's TNF-α, IL-1βdeclined obviously than insulin group ( P <0.05), before treatmen, LADA patient's anti-inflammatory cytokines were lower than the healthy control group ( P <0.05) .Sitagliptin group and in-sulin group's IL-10 and TGF-βlevels increased after treatment, and the difference were statistically significant ( P <0.05), but difference between the 2 groups showed no statistical significance ( P >0.05).Sitagliptin and pro inflammatory factor of△TNF-αand△IL-6 had a significantly negative correlation ( r =-0.718, r =-0.762, P <0.05), and significantly posi-tive correlated with anti-inflammatory factor of△IL-10 and△TGF-β( r =0.677, r =0.712, P <0.05) .Conclusion Sita-gliptin can effectively regulate inflammation and anti-inflammatory system balance in patients with LADA, improve chronic in-flammatory factors'body damage.
