Association of lipid accumulation index with early prediction of abnormal glucose metabolism

Abstract:Objective To analyze the correlation of lipid accumulation index ( LAP) and glucose metabolism.Meth-ods Selected type 2 diabetes and pre diabetes ( including fasting blood glucose regulation and impaired glucose tolerance ( IGT) 238 cases from July 2014 to July 2015 treated in Beijing Ditan Hospital, these patients including pre diabetes group of 113 patients, type 2 diabetes mellitus group of 125 cases, and from Beijing Anzhen Hospital physical examination center, 121 cases of health exam population were enrolled as healthy control group.Physical examination was performed on all subjects, and recorded the sex, age, height, body mass, body mass index, waist circumference, systolic blood pressure, diastolic blood pressure ( DBP) and biochemical data:fasting plasma glucose ( FPG) , fasting insulin, fasting C peptide, OGTT 2 h glucose (2hPG), blood lipids and other biochemical indicators, and according to the formula to calculate insulin resistance index ( HOMA-IR) , LAP.Results Body mass index ( BMI) , waist circumference ( WC) , fasting blood glucose, postprandial 2 hours blood glucose, glycosylated hemoglobin (HbA1c), triglyceride glycerol (TG), insulin resistance index, lipid accumula-tion index increased with the increase of the severity of glucose metabolism ( P <0.05), and high density lipoprotein choles-terol ( HDL-C) showed decreasing trend ( P <0.05) , the difference of total cholesterol between the 3 groups had no statisti-cal significance ( P >0.05).LAP and FPG, 2h PG, HbA1c, TG were significantly positive correlation ( P <0.05), and showed a negative correlation with HDL-C ( P <0.05), but no correlation with TC( P >0.05).Multivariate logistic regres-sion analysis showed that after adjusting for age, gender, when lipid accumulation index increased for each additional 1 stan-dard deviation, glucose metabolism abnormal and type 2 diabetes relative risk were increased with 1.385 (1.109-1.732)and&nbsp;1.947 (1.523-2.488) times;when adjusted for smoking, drinking, blood pressure, TG, TC,HDL-C, suffering from type 2 diabetes relative risk was 1.475 (1.127-1.928);when adjusted for BMI, LAP increased for each additional 1 standard de-viation, type 2 diabetes risk increased 0.438 fold.Conclusion The lipid accumulation index is closely related to the abnor-mal glucose metabolism, and can predict the abnormal glucose metabolism in the early stage.With the increase of lipid accu-mulation index, the risk of type 2 diabetes increased, suggesting that the lipid accumulation index may be an independent risk factor for type 2 diabetes.
