Effect of standard large bone flap craniotomy in treatment of patients with craniocerebral trauma complicated with intracranial hypertension

Abstract：Objective To investigate the clinical effect of standard large trauma craniotomy and traditional craniotomy in the treatment of craniocerebral trauma complicated with intracranial hypertension .Methods from January 2012 to Decem-ber 2013 , 56 patients with craniocerebral trauma complicated with malignant intracranial hypertension were selected .Patients were randomly divided into observation group and control group with 28 cases in each group .The control group was treated with conventional craniotomy , and large bone flap craniotomy was used in the observation group .To compare and analyze the treatment effect of 2 groups of patients.Results Observation group's good recovery rate was 75.00%, significantly higher than that of 46.42% in the control group, the differences were statistically significant ( P <0.05), but the mortality rate was 3.57%, which was significantly lower than that in the control group (14.29%), but the difference was not statistically signif-icant ( P >0.05).In the observation group, the language barrier level and the degree of motor impairment was lower than that in the control group, the differences were statistically significant ( P <0.05).In observation group after 1, 2, 3 and 7 days,intracranial pressure were (24.6 ±2.4) mmHg and (22.0 ±2.5) mmHg and (20.1 ±2.5) mmHg and (15.7 ±3.5 mmHg),respectively, which were significantly lower than the control group (30.9 ±3.8)mmHg, (29.0 ±3.1) mmHg and (27.0 ±3.5) mmHg and (22.2 ±3.4) mmHg, and the difference were statistically significant ( P <0.05).Conclusion Large bone flap craniotomy in the treatment of craniocerebral trauma combined with malignant intracranial hypertension can ef -fectively reduce the intracranial pressure , improve the prognosis of patients , reduce the mortality of patients .
