Effect of tiotropium on prognosis and serum procalcitonin in stable patients with moderate to severe chronic obstruc-tive pulmonary disease

Abstract:Objective To observe the effect of tiotropium on prognosis and serum procalcitonin ( PCT) in patients with stable moderate to severe chronic obstructive pulmonary disease (COPD). Methods From April 2013 to June 2015,102 cases of COPD patients according to the order of admission were randomly divided into two groups, observation group (51 pa-tients) and control group (51 cases). 2 groups of patients were received conventional therapy, inhalation aerosol butanol, in-fection with antibiotics; the observation group also received tiotropium inhaler 20 μg. Lung function before and after treat-ment, 4 min walking distance, dyspnea score and PCT levels, the condition changes during treatment and adverse reactions were compared between the two groups. Results The total exacerbation incidence in the observation group were 5. 88%, which were lower than the control group (45. 10%) (χ2 =20. 648, P <0. 05), after treatment, observation group FVC was (3. 21 ± 0. 71) L, which were significantly better than the control group (2. 43 ± 0. 43) L ( t =6. 711, P <0. 01), FEV1 was (1. 92 ± 0. 43) L, which were significantly better than the control group of (1. 31 ± 0. 31) L ( t =8. 218, P =0. 000), IC was (2. 93 ± 0. 73) L, which was significantly better than the control group (1. 95 ± 0. 52) L ( t =7. 809, P =0. 000), the observation group’s 4 min walking distance and dyspnea scores were (397. 34 ± 40. 12) m and (1. 42 ± 0. 91) points, which were better than the control group’s (291. 45 ± 31. 24) m, (2. 03 ± 0. 58) points, serum PCT was (0. 12 ± 0. 02)μg/L, which was lower than the control group’s (0. 21 ± 0. 04) μg/L, the observation group improved significantly than in the control group ( P <0. 05). Dry mouth found in two cases of the observation group, with well tolerated, and no serious ad-verse reactions of other types; dry mouth patients in the control group were eight cases, three cases of hand-shaking, heart palpitations in one case, two groups’ difference was statistically significant (3. 9 % vs. 23. 5%,χ2 = 8. 279, P <0. 05).&nbsp;Conclusion It demonstrated that the tiotropium reduce procalcitonin in patients in stable moderate to severe chronic obstruc-tive pulmonary disease, the prognosis is good, and it is widely used.
