Initial diagnosis of stroke:a clinical analysis of 11 cases of patent foramen ovale

Abstract:Objective To investigate the diagnosis and the causes of misdiagnosis of patent foramen ovale( PFO) in stroke. Methods From January 2011 to January 2014, retrospective analysis 159 cases of stroke hospitalized patients’ clini-cal data, they were routinely excluded hypertension, history of atherosclerotic disease and dyslipidemia, cardiac emboli, pe-ripheral vascular disease, vasculitis, hypercoagulable state and cause of stroke, no positive findings as"cryptogenic stroke", further using transthoracic echocardiography ( TTE) , transesophageal echocardiography ( TEE) and cardiac pulmonary angiog-raphy ( CTA) examination to exclude PFO. Results In patients with stroke as the first diagnosis, 18 cases were unable to confirm the cause of stroke, further examination showed that the PFO were found in 11 cases, TTE was found in 1 case, TEE+ acoustic angiography confirmed in 9 cases, CTA confirmed 1 case. All PFO were performed with occlusion or operation treatment:10 cases received TEE guided percutaneous PFO closure, 1 case with PFO ride cross thrombus received directly cardiopulmonary bypass under thoracoscope thrombus removed +PFO closure operation;postoperative heart function in all pa-tients recovery well, stroke symptoms relieved or disappeared and discharged from hospital, anticoagulation treatment for 3 months, followed up for 6 months revealed that PFO did not recurrence. Conclusion In stroke patients, especially in crypto-genic stroke, should exclude the existence of PFO.
