Analysis of risk factors of early neurological deterioration in patients with cerebral hemorrhage in basal ganglia

Abstract:Objective To explore the influence factors of early neurological deterioration( END) in patients with cer-ebral hemorrhage in basal ganglia. Methods 82 cases with basal ganglia hemorrhage from March 2012 to June 2014 in de-partment of neurology were selected and divided into END group and non-END group,there were 40 cases in END group and 42 cases in non-END group. Then compared those general clinic date including age, gender, body weight, hypertension, dia-betes, hyperlipidemia, stroke history, smoking history, onset to admission time, NIHSS score, and those laboratory date in-cluding white blood cell count ( WBC) , fasting plasma glucose ( FPG) , prothrombin time ( PT) , fibrinogen ( Fib) , activated partial thromboplastin time ( APTT) , homocysteine ( Hcy) , C-reactive protein ( CRP) , tumor necrosis factor alpha ( TNF-α) and interleukin 6 (IL-6), volume of intracranial hematoma, whether the bleeding breaking into ventricle or not. When a sin-gle factor analysis showed statistical significance, multivariate Logistic regression model analysis was performed to determine the independent risk factors for END in patients with cerebral hemorrhage in basal ganglia region. Results The NIHSS score, WBC count,intracerebral hemorrhage volume, incidence of intraventricular hemorrhage of END group were significantly higher than those of non-END group,there were significant differences( t =8. 863,4. 436,2. 469,11. 149, P <0. 05), and the Lo-gistic regression analysis showed that WBC count(OR=2. 008,95%CI 1. 068~3. 774, P =0. 030),hemorrhage breaking in-to ventricles of brain(OR=3. 675,95%CI 1. 591~8. 486, P =0. 002) were the independent factors for the occurrence of END in patients with cerebral hemorrhage in basal ganglia. Conclusion The higher WBC count and intraventricular hemor-rhage are,the more likely END will happen in patients with cerebral hemorrhage in basal ganglia, and which means a worse outcome for patients.
