Anterolateral extraperitoneal debridement and fusion with internal fixation for the treatment of lumbosacral tubercu-losis

Abstract:Objective To observe the effect of anterolateral extraperitoneal debridement and fusion with internal fixa-tion for treatment of lumbosacral tuberculosis. Methods A retrospective analysis of 21 cases of lumbosacral tuberculosis was performed, one stage extra peritoneal tuberculosis bone fusion and internal fixation were performed on all of these patients, postoperative follow-up of erythrocyte sedimentation rate (ESR), C reactive protein (CRP), X-ray, CT were performed to evaluate the conditions of TB. Results Postoperative follow-up observation was lasted for 9 -48 months, all the incision healed, all cases had no recurrence of tuberculosis. 6-12 months postoperative imaging revealed the fusion of intervertebral bone. At the last follow-up, 1 case’s spinal cord function from preoperative ASIA grade C recovered to grade D, 3 cases from grade D recovered to grade E, ESR from preoperative (38. 1 ± 12. 5) mm/h down to the (11. 3 ± 7. 1) mm/h, CRP from pre-operative (6. 2 ± 4. 2) mg/L down to the (1. 6 ± 1. 3) mg/L, VAS score from preoperative (4. 6 ± 1. 1) points down to the (1. 4 ± 1. 0), preoperative ODI score from (50. 2 ± 11. 9)% down to (13. 0 ± 6. 6)%,lumbosacral angle from preoperative (20. 0 ± 4. 8) degrees to the (29. 0 ± 3. 9) degrees. All the parameter was statistically significant different from before the operation ( P <0. 05). Conclusion Based on the conventional antituberculosis treatment,one stage anterolateral extraperitoneal and radical debridement bone graft fusion and internal fixation for the treatment of lumbosacral tuberculosis is safe and effective.
