Application effect of evidence-based bundle cooling management for patients with heat stroke

Abstract:Objective:This study aimed to investigate the application and efficacy of evidence-based bundle cooling management strategy in the treatment of patients with heat stroke.Methods:The study cohort consisted of patients with heat stroke admitted to the emergency department of our hospital from July 2020 to September 2023.Specifically,46 patients admitted between July 2020 and September 2021 were assigned to the control group,while 48 patients admitted from July 2022 to September 2023 were allocated to the observation group.The control group received standard traditional cooling measures,whereas the observation group,in addition to the routine care,implemented an evidence-based bundle cooling management strategy.This strategy summarized into 6 themes including environmental temperature control,ice packs applied to superficial blood vessels,cooling caps,ethanol rubs combined with fan-assisted cooling,intravenous fluid cooling,and continuous temperature monitoring and management,with a total of 18 pieces of evidence.The study compared the cooling rates,cooling achievement rates,Glasgow Coma Scale(GCS)scores,and mortality rates between the two groups.Results:Following the intervention,the time required for core body temperature to decrease to 39.0℃and 38.5℃was significantly shorter in the observation group than in the control group(all P<0.05).The proportion of patients whose core body temperature was reduced to 39.0℃or below within 30 minutes and to 38.5℃or below within 2 hours was markedly higher in the observation group compared to the control group(all P<0.05).Additionally,the GCS scores at 24 hours post-treatment were significantly higher in the observation group(P<0.05),and the mortality rate was significantly lower in the observation group than in the control group(P<0.05).Conclusion:The implementation of evidence-based bundle cooling management strategy has been demonstrated to significantly enhance the clinical outcomes of heat stroke treatment and improve patient prognosis.
