The impact of early activity programs based on social learning theory on postoperative kinesiophobia in patients undergoing pancreaticoduodenectomy

Abstract:Objective: To explore the application effect of an early activity program based on social learning theory in patients with fear of movement after pancreaticoduodenectomy. Methods: A total of 120 patients who underwent pancreaticoduodenectomy at our hospital from October 2022 to October 2024 were selected as the study subjects and randomly divided into a control group (n=60) and an observation group (n=60) using a random number table method. The control group received routine nursing care, while the observation group received an early activity program based on social learning theory in addition to routine nursing care. The intervention lasted for 3 months. The two groups were compared before and after the intervention in terms of activity self-efficacy, fear of movement, postoperative complications, and recovery conditions. Results: On postoperative day 7, both groups showed increased scores on the General Self-Efficacy Scale (GSES) compared to before the intervention, with the observation group showing higher scores than the control group. The scores on the Tampa Scale for Kinesiophobia (TSK) decreased compared to postoperative day 1, with the observation group showing lower scores than the control group. After the intervention, the incidence of postoperative complications was lower in the observation group than in the control group. The duration of mechanical ventilation, ICU stay, flatus time, defecation time, removal of gastric tube time, initiation of liquid diet time, ambulation time, and hospitalization time were all shorter in the observation group than in the control group, with statistically significant differences (P<0.05). Conclusion: An early activity program based on social learning theory can effectively enhance the activity self-efficacy of patients after pancreaticoduodenectomy, reduce their fear of movement, effectively prevent postoperative complications, and promote postoperative physical function recovery.
