Abstract:Objective: To explore the predictive value of cognitive reserve for post-stroke depression in older adults. Methods: A cross-sectional study design was used, and 306 elderly patients with stroke admitted to the Department of Neurology at Fenyang Hospital of Shanxi Province were selected using convenience sampling from June to November 2023. Patients were divided into depressive and non-depressive groups based on the Hamilton Depression Rating Scale (HAMD-17) and the Montgomery-Åsberg Depression Rating Scale (MADRS). Logistic regression analysis was used to identify factors influencing post-stroke depression, and receiver operating characteristic (ROC) curves were drawn to analyze the predictive value of cognitive reserve for post-stroke depression in older adults. Results: Among the 306 elderly stroke patients, 123 developed post-stroke depression, with a prevalence rate of 40.2%. Multivariate logistic regression analysis showed that the status of the main caregiver, whether it was the first episode, NIHSS score at discharge, geriatric nutritional risk index, and cognitive reserve were significant factors affecting post-stroke depression in older adults (P < 0.05). ROC curve analysis showed that the area under the ROC curve for cognitive reserve in predicting post-stroke depression was 0.708 [95% CI (0.650, 0.767)], with a sensitivity of 69.3% and a specificity of 72.7%, with an optimal cutoff value of 91 points. Conclusion: The cognitive reserve of older adults with post-stroke depression is significantly reduced, and cognitive reserve is an independent factor affecting post-stroke depression, with good predictive value.
