Latent categories analysis of self-management behaviors in patients with chronic obstructive pulmonary disease in the community

Abstract:Objective: To explore the latent classes of self-management behaviors in community patients with chronic obstructive pulmonary disease (COPD) and analyze their influencing factors. Methods: A convenience sampling method was used to select 400 community COPD patients who visited four community health centers affiliated with a tertiary hospital in Nanjing from January 2021 to March 2024. The COPD self-management scale was used to assess the level of self-management behavior at the first visit and 1 month, 3 months, and 6 months after the first visit. A growth mixture model was used to classify self-management behavior categories, and the influencing factors of different categories were analyzed. Results: The self-management behavior of community COPD patients was divided into three latent classes: low-level decline group (25.2%), medium-level increase group (53.7%), and high-level persistent group (21.1%). Multivariate logistic regression analysis showed that age, body mass index (BMI), number of acute exacerbations, lung function, depressive state, social support, and health literacy were significant factors affecting the latent classes of self-management behavior in community COPD patients (P < 0.05). Conclusion: There is heterogeneity in the latent classes of self-management behavior among community COPD patients. Healthcare professionals can develop individualized intervention strategies based on the characteristics and influencing factors of the latent classes of self-management behavior.
