Influencing factors of pregnancy in Chinese postoperative patients with uterine fibroids:a Meta-analysis

Abstract:Objective:To evaluate the influencing factors of pregnancy outcomes in patients after uterine fibroid surgery in China by using Meta-analysis.Methods:Computerized searches were conducted for studies on the influencing factors of pregnancy outcomes in postoperative patients with uterine fibroids in CNKI,WanFang Data,VIP,PubMed,Web of Science,Cochrane Library,and EMbase,all with a search timeframe of the establishment of the database to April 2025.Literature was screened,data extracted and quality evaluated independently by two researchers.Meta-analysis of the included studies was performed using RevMan 5.4 software.Results:A total of 18 articles were included,involving 2 867 patients.Meta-analysis results showed that the number of uterine incisions(RR=0.33,95%CI 0.26-0.42,P<0.000 01),older age(RR=0.39,95%CI 0.35-0.43,P<0.000 01),preoperative pregnancy(RR=0.39,95%CI 0.25-0.61,P<0.000 1),combined infertility(RR=0.44,95%CI 0.31-0.61,P<0.000 01),pelvic adhesions(RR=0.53,95%CI 0.37-0.76,P=0.000 5),years of infertility(RR=0.68,95%CI 0.58-0.79,P<0.000 01),number of leiomyomas(RR=0.71,95%CI 0.57-0.89,P=0.003),leiomyoma size(RR=0.81,95%CI 0.72-0.91,P=0.000 3),and uterine cavity morphology(RR=0.69,95%CI 0.50-0.97,P=0.03),postoperative recurrence(RR=0.70,95%CI 0.54-0.96,P=0.01),TAM(RR=0.76,95%CI 0.66-0.89,P=0.000 6),LM(RR=1.33,95%CI 1.02-1.73,P=0.04),and subplasma leiomyoma(RR=1.59,95%CI 1.34-1.87,P<0.000 01)were the main influencing factors for postoperative pregnancy outcomes in Chinese patients with uterine fibroids(all P<0.05).Conclusion:The postoperative pregnancy outcomes of Chinese patients with uterine fibroids are affected by a multifactorial combination of surgical methods,fibroid characteristics,and the patients'fertility base.Healthcare professionals should better identify high risk groups of patients with adverse pregnancy outcomes based on the above high risk factors and guide their clinical practice in order to increase the success rate of pregnancies and decrease the incidence of adverse pregnancy outcomes in patients with postoperative uterine fibroids.
