Summary of evidence on prevention of oral medication errors in adult inpatients

Abstract:Objective:To systematically retrieve,evaluate,and synthesize evidence related to oral medication errors in hospitalized adults,providing evidence-based guidance for clinical healthcare professionals.Methods:The research team conducted a comprehensive search,meticulous selection,and synthesis of existing literature on preventing oral mediation errors in hospitalized adults.This included domestic and international decision support systems,authoritative guideline websites,specialized databases,and documents including clinical guidelines,systematic review reports,evidence synthesis reports,and expert consensus statements published by official institutions.The search period spanned from database inception to April 1,2025.Two to four researchers independently performed literature screening,quality assessment,evidence extraction,and synthesis.Results:A total of 11 articles were included,comprising 5 guideline documents and 6 systematic review reports.This yielded 38 best evidence statements across 6 themes:order management and decision support,medication management,administration processes,quality management and reporting systems,training systems,and patient education.Conclusion:The evidence summarized in this study for the prevention of oral administration errors in adult inpatients provides an evidence-based basis for the clinical construction of standardized oral administration procedures.It is suggested that medical institutions should optimize the evidence in combination with the local medical environment to reduce the incidence of oral administration errors and ensure the safety of patients'medication.
