Longitudinal study on the fragile development trajectory and influencing factors of pregnant and postpartum women during the perinatal period

Abstract:Objective: To explore the changes in fragility during the perinatal period of pregnant women and analyze the influencing factors of different trajectories. Methods: A convenience sampling method was used to select 150 perinatal women admitted to our hospital between January and December 2024 as the study subjects. At baseline, a general information questionnaire was administered. At gestational weeks 28 (T1), 32 (T2), 37 (T3), the day after childbirth (T4), and one week postpartum (T5), the Fragility Checklist was used for questionnaire surveys. Data analysis was performed using latent class growth modeling and logistic regression analysis. Results: The total scores of the Fragility Checklist at T1-T5 were (110.37±21.37), (118.83±20.70), (125.94±19.34), (120.83±18.28), and (117.38±17.64), respectively. There were statistically significant differences in the total scores of the Fragility Checklist across different time points (F=129.333, P<0.001). Three distinct trajectories of fragility were identified during the perinatal period of pregnant women: high-fragility group (28%), medium-fragility group (33%), and low-fragility group (39%). Logistic regression analysis showed that age, educational level, number of births, and social support were influencing factors of the perinatal fragility development trajectory (P<0.05). Conclusion: There are three potential development trajectories of fragility among perinatal women: high-fragility group, medium-fragility group, and low-fragility group. Different trajectories are influenced by factors such as the age, educational level, number of births, and social support of the pregnant women. For high-risk groups, strengthening health education and psychological counseling, as well as improving the social support network, can help reduce the level of fragility during the perinatal period.
