Application of discharge readiness service program for caregivers of children with henoch-schönlein purpura

Abstract:Objective: To explore the intervention effect of a discharge preparation service program based on timing theory on caregivers of children with allergic purpura (HSP). Methods: A convenience sampling method was used to select 84 caregivers of HSP children hospitalized in a tertiary class A children's hospital in Shanxi Province from June to November 2022 as the study subjects. The caregivers hospitalized from June to August 2022 were assigned to the control group and received routine discharge preparation measures. The caregivers hospitalized from September to November 2022 were assigned to the experimental group and received a discharge preparation service program for HSP caregivers based on timing theory in addition to routine nursing care. The two groups were compared in terms of discharge preparedness and quality of discharge instructions after the intervention. Meanwhile, the caregivers' caregiving ability and disease uncertainty were analyzed before the intervention, on the day of discharge, and 1 and 3 months after discharge. The unplanned readmission rate of the children was also recorded at 1 and 3 months after discharge. Results: Finally, 42 caregivers in the experimental group and 40 caregivers in the control group completed the study. After the intervention, the scores of discharge preparedness and quality of discharge instructions of the experimental group were higher than those of the control group. Moreover, at the time of discharge, 1 month, and 3 months after discharge, the caregivers' caregiving ability in the experimental group improved more significantly than that in the control group, and their disease uncertainty scores were lower than those in the control group. At 1 month and 3 months after discharge, the unplanned readmission rate of the children in the experimental group was lower than that in the control group. The differences were statistically significant (P < 0.05). Conclusion: The discharge preparation service program for caregivers of HSP children based on timing theory can effectively improve the discharge preparedness, quality of discharge instructions, and caregiving ability of HSP caregivers, reduce disease uncertainty, and decrease the unplanned readmission rate of children. It can provide a reference for the discharge preparation work of HSP caregivers.
