Constructing management plan for incontinence-associated dermatitis in neurointensive care patients based on action research method

Abstract:Objective:To develop a management protocol for IAD in neurointensive care patients,to reduce the incidence of IAD in neurointensive care patients.Methods:The convenience sampling method was used,and 96 neurointensive care patients were selected and divided into control group(32 cases),first cycle group(32 cases)and second cycle group(32 cases)according to the length of hospitalization.The action research method was adopted to continuously revise and improve the clinical management scheme of IAD through two cycles.The incidence,occurrence time,severity,healing time of IAD among three groups were compared,and we also compared the IAD knowledge assessment results,the compliance rate of nursing behavior of IAD among three groups.Results:There were statistically significant differences among three groups(P<0.05),including the incidence,occurrence time,healing time,severity of IAD,and the IAD knowledge assessment results,the nursing behaviors compliance rate.Correlation analysis showed that the compliance rates of IAD risk assessment,severity assessment of fecal incontinence,incontinence management measures,skin cleaning and protection measures were negatively correlated with the occurrence and severity of IAD(P<0.05,r value>0.2),and positively correlated with the occurrence time of IAD(P<0.05,r value>0.6),and negatively correlated with the healing time of IAD(P<0.05,r value>0.4).Regression analysis showed that not taking stool regularly was an independent risk factor for IAD,not communicating with the doctor daily was a positive factor for IAD severity,and communicating with the doctor was a negative factor for IAD healing time.Conclusion:The clinical management program of incontinence dermatitis based on action research method is helpful to improve nurses' ability to care for incontinence dermatitis,reduce the incidence of incontinence dermatitis and alleviate patients' pain.
