Mediating effect of anxiety between symptom burden and fear of exercise in elderly patients with chronic heart failure

Abstract:Objective: To understand the current status of symptom burden, anxiety, and exercise fear in elderly patients with chronic heart failure, and to explore the mediating effect of anxiety between symptom burden and exercise fear. Methods: A convenience sampling method was used to select 355 elderly patients with chronic heart failure who were hospitalized in the cardiology department of a tertiary general hospital in Ganzhou City, Jiangxi Province from December 2022 to November 2023 for a cross-sectional survey. The general information questionnaire, Heart Failure Symptom Perception Scale (HFSPS), Generalized Anxiety Disorder scale (GAD-7), and Chinese version of the Test of Self-Reported Exercise Fear Scale (TSK-SV Heart-C) were used for data collection. Results: The total score of HFSPS in elderly patients with chronic heart failure was (20.75±8.52); the total score of GAD-7 was (3.20±3.35); the total score of TSK-SV Heart-C was (41.69±7.09). There was a partial mediating effect of anxiety between symptom burden and exercise fear, with a mediating effect of 0.063, accounting for 14.65% of the total effect. Conclusion: Anxiety plays a mediating role between symptom burden and exercise fear in elderly patients with chronic heart failure. Symptom burden can directly affect exercise fear, and also indirectly influence it through anxiety.
