Current status and risk prediction model construction of frailty and pre-frailty exercise ability in elderly patients with chronic respiratory diseases

Abstract:Objective: To investigate the current status of frailty and pre-frailty physical ability in elderly patients with chronic respiratory diseases (CRD), and to construct a risk prediction model for frailty and pre-frailty in elderly CRD patients. Methods: A total of 380 elderly CRD patients admitted to Zhongshan Traditional Chinese Medicine Hospital from May 2022 to August 2023 were selected as the study subjects. According to their frailty status, they were divided into three groups: the frail group (n=157), the pre-frail group (n=128), and the non-frail group (n=95). The physical abilities of the three groups were compared using the Activities of Daily Living (ADL) scale, Timed Up and Go Test (TUGT), and Morse Fall Scale (MFS). Multivariate logistic regression analysis was used to identify the influencing factors of frailty and pre-frailty, and the receiver operating characteristic (ROC) curve was used to evaluate the predictive performance. Results: The ADL score of the frail group was lower than that of the pre-frail and non-frail groups, while the TUGT and MFS scores were higher than those of the pre-frail and non-frail groups. The ADL score of the pre-frail group was lower than that of the non-frail group, while the TUGT and MFS scores were higher than those of the non-frail group, with statistically significant differences (P<0.05). Multivariate analysis showed that age, polypharmacy, exercise habits, long-term home oxygen therapy (LTOT), sleep disorders, depressive state, pulmonary function classification, and social support were independent risk factors for frailty and pre-frailty in elderly CRD patients. The Hosmer-Lemeshow goodness-of-fit test showed that the risk prediction model had a good fit (P>0.05). The area under the ROC curve was 0.803 [95% CI (0.741, 0.852), P<0.001]. Conclusion: The physical ability of elderly CRD patients with frailty and pre-frailty has significantly decreased. The risk prediction model constructed based on eight influencing factors shows good predictive performance.
