Influence of probiotics combined with enteral nutrition on gastrointestinal and infectious complications in patients with severe stroke:a Meta-analysis

Abstract:Objective:To evaluate the effect of probiotics combined with enteral nutrition intervention on gastrointestinal and infectious complications in patients with severe stroke through Meta-analysis.Methods:RCTs on the effects of probiotics combined with enteral nutrition intervention on gastrointestinal and infectious complications in patients with severe stroke were retrieved from Cochrane Library,PubMed,EMbase,CINAHL,Web of Science,CNKI,VIP,WanFang Data.ProQuest and OpenGrey were retrieved to obtain unpublished research.The retrieval deadline was from the establishment of the database to May 31,2024.The Cochrane Collaboration RoB2 was used for literature quality evaluation.Two researchers independently conducted literature screening,data extraction,and literature quality evaluation.RevMan 5.4 software was used for Meta-analysis.Results:A total of 20 articles were included,involving 1 704 patients with severe stroke.Meta-analysis results showed that probiotics combined with enteral nutrition can reduce the incidence of diarrhea(OR=0.22,95%CI 0.11-0.43,P<0.000 1),bloating(OR=0.51,95%CI 0.28-0.94,P=0.03),constipation(OR=0.44,95%CI 0.22-0.89,P=0.02),gastrointestinal bleeding(OR=0.26,95%CI 0.14-0.47,P<0.000 1),pulmonary infections(OR=0.39,95%CI 0.28-0.53,P<0.000 01),urinary tract infections(OR=0.21,95%CI 0.13-0.35,P<0.000 01),and gastrointestinal infections(OR=0.23,95%CI 0.11-0.46,P<0.000 1)in patients with severe stroke.Conclusions:Existing evidence suggests that probiotics combined with enteral nutrition intervention can reduce the incidence of gastrointestinal complications(diarrhea,bloating,constipation,gastrointestinal bleeding)and infectious complications(pulmonary infection,urinary tract infection,gastrointestinal infection)in patients with severe stroke.
