Effect of non-drug intervention on improving cognitive function in patients with mild cognitive impairment in Parkinson's disease:a network Meta-analysis

Abstract:Objective:To explore the effect of different non-drug interventions on improving mild cognitive impairment(MCI)in patients with Parkinson's disease.Methods:Randomized controlled trials(RCTs)on the effect of non-drug interventions in MCI patients with Parkinson's disease were retrieved from domestic and foreign databases.The time limit was from the establishment of the database to July 8,2024.Stata 15.1 and Review Manager 5.4 software were used for network Meta-analysis.Results:A total of 20 articles were included,involving 1 148 MCI patients with Parkinson's disease and 11 non-drug intervention methods.The network Meta-analysis results showed that the surface under the cumulative ranking(SUCRA)of non-drug intervention effects of 8 studies using the Mini-Mental State Examination(MMSE)score as the evaluation index showed that virtual reality(VR)combined with repetitive transcranial magnetic stimulation(rTMS)>cognitive training combined with rTMS>rTMS>moxibustion treatment>acupuncture treatment>exercise intervention>cognitive training combined with transcranial direct current stimulation(tDCS)>cognitive training>conventional intervention>tDCS.The non-drug intervention effect SUCRA of 18 studies using the Montreal Cognitive Assessment(MoCA)score as the evaluation index showed that VR combined with rTMS>dual-task training>cognitive training combined with rTMS>acupuncture treatment>rTMS>cognitive training>VR training>health education>exercise intervention>conventional intervention.VR combined with rTMS had the highest SUCRA in improving MMSE and MoCA scores.Conclusions:Existing evidence shows that VR combined with rTMS may be the best non-drug intervention method to improve cognitive function in MCI patients with Parkinson's disease.Cognitive training combined with rTMS,dual-task training,and acupuncture therapy may be more effective intervention methods to improve their cognitive function.However,due to the limitations of the number and quality of included literature,more high-quality RCTs are needed to further verify the conclusions of this study in the future.
