Abstract:Objective: To explore the trajectory changes in medication adherence of patients with primary hypertension who were hospitalized for the first time based on the Health Ecological Model, and to identify influencing factors, providing a reference for developing targeted intervention measures. Methods: A convenience sampling method was used to select 370 patients with primary hypertension who were hospitalized for the first time at a tertiary A-grade general hospital in Beijing from January 2020 to June 2023 as the study subjects. The Morisky Medication Adherence Scale was used for seven surveys at admission and 1, 2, 3, 4, 5, and 6 months after discharge to describe the trajectory of medication adherence. A multivariate, multilevel repeated measures ANOVA was used to explore the influencing factors of medication adherence in hypertensive patients. Results: The trajectory of medication adherence gradually increased from admission to one month after discharge, rapidly decreased between two to three months after discharge, and then slowly declined after three months. Multivariate analysis results showed that the influencing factors of medication adherence trajectories included age and self-efficacy in the personal characteristics layer, social support in the interpersonal relationships layer, and smoking and drinking habits in the behavioral characteristics layer. Conclusion: Influencing factors of medication adherence in patients with primary hypertension exist in multiple dimensions of the Health Ecological Model. Medical staff should pay attention to young patients with low self-efficacy and insufficient social support, as well as those with smoking and drinking habits, and implement targeted interventions according to the influencing factors to improve medication adherence in hypertensive patients.
