Construct health promotion behavior intervention scheme for patients with chronic heart failure based on Triangle model

Abstract：Objective: To construct a health promotion behavior intervention program for patients with chronic heart failure based on the Triangle model, providing a basis for nursing staff to effectively manage chronic diseases. Methods: Taking the Triangle chronic disease stratification and grading model as the theoretical foundation, the initial draft of the health promotion behavior intervention program for patients with chronic heart failure was formulated through literature analysis. A two-round Delphi method was used to consult 15 experts, forming the health promotion behavior intervention program for patients with chronic heart failure. Results: The recovery rates of the questionnaires in the two rounds of expert consultation were 75.0% and 100.0%, respectively. The authority coefficients of the experts were 0.953 and 0.960, respectively. The Kendall's W values were 0.259 and 0.344 (P < 0.001). The importance scores of each item ranged from 4.07 to 5.00, with a coefficient of variation ranging from 0.00 to 0.20. The final constructed health promotion behavior intervention program for patients with chronic heart failure includes 5 first-level indicators, 15 second-level indicators, 39 third-level indicators, and 24 fourth-level indicators. Conclusion: Constructing a stratified and graded health promotion behavior intervention program for patients with chronic heart failure based on the Triangle model is scientific and feasible, and can provide clinical guidance for the management of patients with chronic heart failure outside the hospital.
