Risk factors for carbapenem-resistant enterobacterales infection in adult ICU patients:a Meta-analysis

Abstract:Objective:To explore the risk factors of carbapenem-resistant enterobacteriaceae(CRE)infection in adult patients in the ICU through Meta-analysis.Methods:the related studies on the risk factors of CRE infection in adult ICU patients were retrieved from PubMed,MedLine,the Cochrane Library,Web of Science,EMbase,VIP,CNKI,CBM,and WanFang Database.The search period was from the establishment of the database to September 20,2024.Literature screening,data extraction and literature quality evaluation were independently conducted by two researchers.RevMan 5.0 and Stata 16.0 software were adopted for Meta-analysis and publication bias tests.Results:A total of 18 articles were included,which 16 were in Chinese and 2 in English.The results of the Meta-analysis showed that having underlying pulmonary diseases(OR=2.83,95%CI 1.37-5.83,P=0.005),indwelling gastric tubes(OR=5.51,95%CI 3.79-8.02,P<0.000 01),mechanical ventilation assistance(OR=2.74,95%CI 1.49-5.05,P=0.001),the use of carbapenem antibiotics(OR=3.82,95%CI 3.11-4.69,P<0.000 01),the use of aminoglycoside drugs(OR=2.94,95%CI 1.66-5.22,P=0.000 2),the use of antifungal drugs(OR=2.90,95%CI 1.96-4.29,P<0.000 01),the simultaneous use of two or more antibiotics(OR=3.64,95%CI 2.12-6.24,P<0.000 01),and APACHE Ⅱ score(qualitative data)(OR=2.63,95%CI 1.69-4.10,P<0.000 01),APACHE Ⅱ score(quantitative data)(OR=5.73,95%CI 2.39-13.73,P<0.000 01)were risk factors for CRE infection in ICU patients.Conclusion:Existing evidence indicates that having underlying lung diseases,indwelling gastric tubes,mechanically assisted ventilation,the use of carbapenem antibiotics,the use of aminoglycoside drugs,the use of antifungal drugs,the simultaneous use of two or more antibiotics,and a high APACHE Ⅱ score are risk factors for CRE in ICU patients.Effective preventive measures should be taken to reduce the infection rate of CRE.
