Status and influencing factors of time in range in elderly patients with type 2 diabetes mellitus

Abstract:Objective: To analyze the current status of time in range (TIR) for glucose in elderly patients with type 2 diabetes and its influencing factors, and to explore the predictive value of psychosocial factors. Methods: A convenience sampling method was used to select 157 elderly patients with type 2 diabetes who were treated at our hospital between January 2022 and January 2024. Data were collected using a clinical information questionnaire and a diabetes-related problem scale. Logistic regression analysis was used to investigate the influencing factors of TIR for glucose in elderly patients with type 2 diabetes, and further tests were conducted to examine the predictive value of psychosocial factors on TIR. Results: The average TIR for glucose in 157 elderly patients with type 2 diabetes was 65.22%. Pearson correlation analysis showed that TIR for glucose was negatively correlated with total psychological distress score (r = -0.415, P < 0.01), and positively correlated with total scores and each dimension of self-management knowledge, attitude, and practice (r values from 0.357 to 0.646, all P < 0.01), as well as with total self-management efficacy score and each dimension score (r values from 0.415 to 0.873, all P < 0.001). Logistic regression analysis showed that age, presence of comorbidities, receipt of blood glucose management education, psychological distress, self-management knowledge, attitude, and practice, and self-management efficacy were influencing factors for TIR in elderly patients with type 2 diabetes (P < 0.001). The areas under the receiver operating characteristic (ROC) curve (AUC) for predicting TIR were 0.726 for psychological distress, 0.779 for self-management knowledge, attitude, and practice, and 0.717 for self-management efficacy. Conclusion: Age, presence of comorbidities, receipt of blood glucose management education, psychological distress, self-management knowledge, attitude, and practice, and self-management efficacy are influencing factors for TIR in elderly patients with type 2 diabetes, and psychosocial factors such as psychological distress have good predictive value for TIR.
