Constructing an evaluation index system for the competence of community nurses in nursing for common chronic diseases in the elderly based on the iceberg model

Abstract:Objective: To construct a competency evaluation index system for community nurses in the care of common chronic diseases in the elderly based on the iceberg model, providing a reference for developing standardized and homogenized training strategies and assessment criteria for community chronic disease nursing positions. Methods: Based on the iceberg model as a theoretical framework, the study used literature review and the Delphi expert consultation method to build the competency index system for community nurses in the care of common chronic diseases in the elderly. The analytic hierarchy process (AHP) was applied to quantitatively analyze the dimensions, items, and evaluation indicators at each level, and determine the weights of each indicator. Results: After two rounds of expert consultation, the final index system included four first-level indicators, twelve second-level indicators, twenty-eight third-level indicators, and sixty-six explanations of third-level indicators. The authority coefficients of the expert questionnaires in the two rounds were 0.786 and 0.827, respectively, and the Kendall's coefficient of concordance were 0.141 and 0.133, indicating that the constructed index system has good consistency. Conclusion: The competency evaluation index system for community nurses in the care of common chronic diseases in the elderly established in this study is scientific, reliable, and comprehensive, and can provide quantitative basis for the access, certification, training, and assessment of community chronic disease nurses, with both theoretical and practical significance.
