Development of review indicators and analysis of obstacle factors for non-pharmacological intervention to promote postoperative gastrointestinal function recovery in gynecological surgery

Abstract:Objective:To implement evidence-based practice of non-pharmacological intervention so as to promote postoperative gastrointestinal function recovery in gynecological surgery,develop review indicators,analyze barriers and facilitators in evidence-based nursing practice,and develop action strategies.Methods:Based on the Joanna Briggs Institute(JBI)Clinical Evidence Application Model,the clinical nursing problem was identified,an evidence-based nursing practice team was formed,evidence was systematically searched,evaluated,and summarized,and corresponding review indicators and methods were developed.A baseline review was conducted from December 12,2022,to January 30,2023.Based on the results of the baseline review,barriers and facilitators in the evidence-based practice process were analyzed,and change strategies were developed.Results:Totally 17 pieces of best evidence were included and 16 review indicators were developed.There were 6 indicators with accurate execution rate of<60%,and 5 of them were 0.The main barriers were the lack of attention and relevant knowledge of nurses to assessment energy intake,lack of assessment process in energy management,specific dietary management,activity assessment and planning table,and related educational materials in the department,as well as a lack of personnel and management process for ear acupressure technique implementation.The main facilitators were strong organizational leadership,collaboration among medical and nursing teams,and support from multidisciplinary teams.Corresponding change strategies were developed based on these factors.Conclusion:The review indicators constructed based on the best evidence and professional judgment in this study are scientific,effective,appropriate,and feasible.The analysis of barriers and facilitators and the development of change strategies can provide personalized non-pharmacological intervention nursing plans for postoperative gastrointestinal function recovery in gynecological patients and guarantee clinical practice.
