Summary of best evidence and evaluation of evidence availability for management of high-risk diabetic foot patients in communities

Abstract:Objective:To retrieve,evaluate,and summarize the best evidence on foot management for high-risk diabetic foot patients in the community and evaluate the evidence availability,so as to provide an evidence-based basis for foot management of high-risk diabetic foot patients in the community.Methods:Based on the"6S"model,evidence on foot management for high-risk diabetic foot patients,including clinical decision-making,guidelines,recommended practices,systematic reviews,expert consensus,and evidence summaries,were retrieved systematically from databases in China and foreign countries.The search time limit was from the establishment of the database to October 31,2022.According to the evaluation standards established by the Clinical Research and Evaluation System(AGREE Ⅱ)and the Australian Joanna Briggs Institute(JBI)Evidence-Based Health Care Center(2016),two researchers independently assessed the quality of the literature,extracted evidence from the literature that met the quality standards,completed the evidence grading using the 2014 version of the JBI evidence pre-grading system,and screened out the evidence available in the community using the evidence availability assessment.Results:A total of 16 articles were included,including 7 guidelines,2 expert consensus articles,5 systematic reviews,1 clinical decision-making article,and 1 evidence summary.A total of 40 best pieces of evidence in 11 aspects were summarized,including foot screening,multidisciplinary management,exercise assessment,exercise purpose,exercise methods and effects,exercise time and frequency,exercise intensity,foot care,wearing shoes and socks,foot health education,and follow-up monitoring.Finally,37 pieces of evidence suitable for community transformation were selected.Conclusions:This study summarizes the best evidence on managing high-risk diabetic feet and fully considered the community context to evaluate the availability of evidence.It can provide a reference for managing high-risk diabetic foot in the community and lay the foundation for evidence transformation.
