The predictive value of AIS-APS scale for stroke-associated pneumonia in patients with ischemic stroke:a Meta-analysis

Abstract:Objective:To evaluate the predictive value of AIS-APS scale for stroke-associated pneumonia(SAP)in patients with ischemic stroke systematically.Methods:The relevant literature using AIS-APS scale to predict SAP risk of ischemic stroke was retrieved from CNKI,WanFang Database,VIP,CBM,PubMed,Web of Science,EMbase,the Cochrane Library and Wiley.The retrieval time was from the inception to May 31,2023.QUADAS-2 was used for literature quality evaluation,and Stata 17.0 software was used for Meta-analysis.Results:14 articles were included in the Meta-analysis,with a total of 7 117 patients.Meta-analysis results showed that AIS-APS predicted SAP co-sensitivity of ischemic stroke was 0.82(95%CI 0.74-0.88),co-specificity was 0.73(95%CI 0.66-0.80),and co-positive likelihood ratio was 3.08(95%CI 2.53-3.76).The combined negative likelihood ratio was 0.25(95%CI 0.18-0.34),and the combined diagnosis ratio DOR was 2.52(95%CI 2.20-2.84),combined odds ratio was 12.40(95%CI 9.01-17.06).AIS-APS predicted SAP in ischemic stroke with an area under the curve(AUC)of SQOC was 0.84(95%CI 0.81-0.87).Deek's funnel plot analysis showed no publication bias in the included literature(P=0.73),and Van Root plot showed that the score was clinically applicable.Conclusion:Current evidence Shows that AIS-APS scale has a certain predictive value for the risk of SAP in patients with ischemic stroke,which can be used for preliminary screening of clinical patients and identification of high-risk patients with SAP for further prevention and treatment.
