Abstract:A 54-year-old female patient presented with unclear speech and facial droop for 2 months. She was admitted to the hospital on October 7, 2024. Two months prior (August 2024), she developed unclear speech and facial droop without obvious precipitating factors. Her limb movements were normal, and she had no chest tightness, shortness of breath, fever, dizziness, headache, or limb convulsions. A head MRI at another hospital showed severe stenosis of the right internal carotid artery (ICA) and an infarct in the left basal ganglia. She received conservative treatment (specific regimen unknown), but her symptoms did not improve. For further diagnosis and treatment, she was referred to our neurosurgery department. Head and neck CTA showed that the right internal carotid artery originated from the brachiocephalic artery, with severe stenosis at the origin and a thin entire course (Figure 1). She was admitted to the outpatient clinic with a diagnosis of 'severe stenosis of the right internal carotid artery'. Since the onset of the disease, her mental state, sleep, and diet have been good, without fever, no cough during eating or drinking, normal urination and defecation, and no significant weight change. She has a history of hypertension for 1 year, with a maximum blood pressure of 200/130 mm Hg (1 mm Hg = 0.133 kPa), taking telmisartan 40 mg/day regularly, with blood pressure controlled at 130/90 mm Hg; she also has a history of diabetes for 1 year, taking empagliflozin 10 mg/day and metformin 500 mg/day regularly, with blood glucose controlled between 7.50–8.00 mmol/L; there are no special personal or family histories.
