Relationships between diabetic peripheral neuropathy and affective factors
Abstract:The development of diabetic peripheral neuropathy is af fected by many factors and it is well known that hyperglycemia is the most significant one of the factors.OBJECTIVE: To illustrate the importance of glycemic control and the relationship between diabetic peripheral neuropathy and each relative factor.DESIGN: A retrospective controlled study based on diagnosis.SETTING and PARTICIPANTS: Totally 120 diabetic in-patients (according to standards in 1999) of the department of endocrinology in our hospital were collected from June 1999 to June 2001.METHODS:Nerve conductive velocity(NCV) in 120 diabetic patients was determined. Step gradual regression analyses were performed between NCV and all factors. NCV was divided into groups at different HbAlc levels, and t test was used.MAIN OUTCOME MEASURE: Every nerve conductive velocity, course of disease, fasting blood sugar, post-prandial blood sugar, age, total cholesterol,triglyceride, HbAlc, FA, C-peptide, post-prandial peptide and 24 hours urinary protein were examined.RESULTS: Among the relative factors, the most significant difference was observed in HbAlc (4 items), then in age (3 items), FBS and the course of disease(2 items) respectively. The first significant change of NCV was observed when HbAlC reached 6. 8. Significant changes were observed in all NCV when HbAlc was greater than 8.0. The same changes happened when HbAlc was greater than 9.0, when the factors of the age and the course were eliminated.CONCLUTION: Among the factors related to diabetic peripheral neuropathy, HbAlc expresses the closest correlation. The course and the age also played the extremely vital role.
