Effects of Combined Pelvic Floor Electrical Stimulation and Myofascial Release Therapy on Pelvic Floor Muscle Strength and Stress Urinary Incontinence in Patients with Pelvic Floor Disorders(PFD)

Abstract:Objective:To investigate the effect of a pelvic floor dysfunction(PFD)treatment device combined with pelvic floor myofascial manual therapy on pelvic floor muscle strength and stress urinary incontinence in patients with pelvic floor dysfunction.Methods:A total of 114 PFD patients treated at the 12th People's Hospital of Guang-zhou from October 2018 to September 2023 were randomly divided into three groups,each with 38 patients.All groups received conventional rehabilitation training.Additionally,group A received the PFD treatment device,group B received pelvic floor myofascial manual therapy,and the combined group received both the PFD treatment device and pelvic floor myofascial manual therapy.The clinical efficacy,pelvic floor muscle electromyography(EMG)be-fore and after intervention,modified Oxford scale for pelvic floor muscle strength,stress urinary incontinence,pelvic floor function,and sexual function[Pelvic Floor Impact Questionnaire-Short Form(PFIQ-7),Female Sexual Func-tion Index(FSFI),Pelvic Floor Distress Inventory-Short Form(PFDI-20)],and social impact[Social Impact Scale(SIS)]were evaluated.Results:After 8 weeks of intervention,the total effective rate in the combined group[92.11%(35/38)]was higher than in group A[71.05%(27/38)]and group B[65.79%(25/38)].The proportion of urinary incontinence in the combined group[5.26%(2/38)]was lower than in group A[23.68%(9/38)]and group B[28.95%(11/38)],and the differences were statistically significant(P<0.05).However,there were no sta-tistically significant differences between group A and group B(P>0.05).After 8 weeks of intervention,the mean an-terior and posterior resting EMG values and the average number of urinary incontinence episodes over 24 hours were lowest in the combined group,followed by group A and then group B.The type Ⅰ and type Ⅱ fiber potentials and modified Oxford scale scores for pelvic floor muscle strength were highest in the combined group,followed by group A and then group B.The FSFI scores were highest in the combined group,followed by group A and then group B.The PFIQ-7,PFDI-20,and SIS scores were lowest in the combined group,followed by group A and then group B,and the differences were statistically significant(P<0.05).Conclusion:The combination of a PFD treatment device and pelvic floor myofascial manual therapy is effective in improving pelvic floor muscle strength and reducing stress urinary incontinence and sexual dysfunction in PFD patients,thereby promoting better pelvic floor function and reduc-ing the sense of stigma.
