Application of the Modified Peyton's Four-Step Teaching Method in Simulation Training for Basic Laparoscopic Skills:An Evidence-Based Analysis

Abstract:Objective:To investigate the efficacy of the modified Peyton's four-step teaching method in simula-tion training for basic laparoscopic skills.Methods:Forty resident physicians undergoing standardized training in the Department of Urology at the First Affiliated Hospital of the University of Science and Technology of China(Anhui Provincial Hospital)between July 2020 and July 2022 were enrolled and randomly assigned to two groups:the study group(n=20,modified Peyton method)and the control group(n=20,traditional didactic training).After a 4-week intervention,both groups underwent skill assessments using standardized tasks,and teaching satisfaction was evaluat-ed via questionnaires.Statistical analyses included Student's t-test and Mann-Whitney U test.Results:Compared with the control group,the study group demonstrated shorter operation times in clip transfer[(79.09±12.08)s vs.(92.81±18.86)s,t=-2.740,P=0.010]and suture knot-tying[(134.55±26.15)s vs.(159.26±28.31)s,t=-2.867,P<0.001)],the differences were statistically significant(all P<0.05).However,there was no significant difference was observed in the circle-cutting task[(263.17±34.59)scores vs.(277.93±57.76)scores,t=-0.980,P=0.333)]between the two groups.Regarding operative quality,the study group achieved significantly higher GOALS scores than the control group in all tasks:clip transfer(21.45±4.58 vs.17.3±2.98,t=3.397,P=0.002),circle-cutting(18.85±4.74 vs.16.15±2.83,t=2.187,P=0.035),and suture knot-tying(20.85±4.73 vs.17.95±3.95,t=2.105,P=0.042),all of the differences were statistically significant.The questionnaire survey showed that the stu-dents in the study group were significantly higher than those in the control group in terms of improving learning enthu-siasm,improving class participation,improving learning efficiency,improving teaching order and teaching satisfac-tion(all P<0.05).Conclusion:The modified Peyton's teaching method demonstrates superior efficacy and learner satisfaction in laparoscopic simulation training,supporting its broader implementation in surgical education programs.
