Clinical Efficacy Analysis of Transvaginal 'Lap-Overlay' Technique versus Laparoscopic Repair for Vesicovaginal Fistula

Abstract:Objective:To compare the clinical efficacy of the transvaginal"lap-overlay"technique versus laparo-scopic repair for vesicovaginal fistula(VVF).Methods:A retrospective analysis was conducted on 80 patients with VVF treated at the Department of Urology,Xuzhou Central Hospital and West China Hospital of Sichuan University between March 2018 and March 2023.Based on the surgical approach,patients were divided into two groups:the transvaginal group(n=40),treated with the"lap-overlay"technique,and the transabdominal group(n=40),treated via laparoscopic repair.Operative time,intraoperative blood loss,hospital stay,and postoperative complications were compared between the two groups.Results:Statistical analysis was performed using SPSS 26.0.Normally dis-tributed continuous data were expressed as mean±standard deviation((x)±s)and analyzed using the t-test.Categori-cal data were presented as frequency and percentage[n(%)]and compared using the x2 or Fisher's exact test.A P-value<0.05 was considered statistically significant.The transvaginal group demonstrated significantly shorter opera-tive time(110±12 min vs.214±11 min,P<0.05),less intraoperative blood loss(21±4 mL vs.182±37 mL,P<0.05),and shorter hospital stay(6.9±1.2 days vs.12.0±1.3 days,P<0.05)compared to the transabdominal group.The postoperative complication rates were 7.5%(3/40)and 10.0%(4/40)in the transvaginal and transabdominal groups,respectively.All patients were followed up for 3-6 months,with recurrence rates of 5.0%(2/40)in the transvaginal group and 7.5%(3/40)in the transabdominal group.Conclusion:Compared to laparoscopic repair,the transvaginal"lap-overlay"technique for VVF repair offers advantages including minimal invasiveness,shorter opera-tive time,and reduced hospitalization.It is a safe and effective minimally invasive surgical approach.
