Comparative Analysis of UMP versus RIRS for Renal/Proximal Ureteral Calculi:Stone Clearance Rates,Safety Profiles,and Effects on Inflammatory Markers and Renal Function

Abstract：Objective:To compare the efficacy,safety,and impact on inflammatory markers and renal function between ultra-mini percutaneous nephrolithotomy(UMP)and retrograde intrarenal surgery(RIRS)for the treatment of renal/proximal ureteral calculi.Methods:A retrospective study was conducted on 97 patients with renal/proximal ureteral calculi treated at Tongling People's Hospital and the First Affiliated Hospital of Bengbu Medical University between March 2023 and March 2024.Patients were divided into two groups:the UMP group(n=45)and the RIRS group(n=52).Perioperative outcomes,stone clearance rates,inflammatory markers(TNF-α,IL-6,CRP),renal function parameters(BUN,Scr,Cys-C),and safety profiles were compared.Results:The UMP group exhibited significantly higher intraoperative blood loss[(27.67±6.37)ml vs.(10.57±3.11)ml,P＜0.05)]and intraoperative injury rates(20.00％vs.5.77％,P＜0.05)but lower renal pelvic pressure(12.21±3.22)mmHg vs.(22.15±4.67)mmHg,P＜0.05)compared to the RIRS group.No significant difference was observed in operative time(P＞0.05).The UMP group achieved a higher stone clearance rate(91.11％[41/45]vs.73.08％[38/52],x2=5.191,P=0.023).Postoperatively,the RIRS group showed more pronounced elevations in inflammatory markers(TNF-α,IL-6,CRP)and renal function indicators(BUN,Scr,Cys-C)(P＜0.05).Complication rates had no significantly differ-ence between groups(P＞0.05).Conclusion:For renal/proximal ureteral calculi(≤2.5 cm),both UMP and RIRS are effective treatment options.UMP demonstrates superior stone clearance but carries higher risks of bleeding and tis-sue injury,whereas RIRS offers minimal invasiveness and faster recovery at the cost of lower stone-free rates and greater inflammatory response.
