Application of Ultrasonography Combined with Flexible Ureteroscopy in the Treatment of Parapelvic Renal Cysts

Abstract:Objective:To investigate the potential ad van tages and limitations of ultrasound-guided flexible ure-teroscopy in the management of parapelvic renal cysts.Methods:A retrospective analysis was conducted on 75 pa-tients with parapelvic cysts treated at the Department of Urology,Yongchuan Hospital of Chongqing Medical Univer-sity,between January 2018 and January 2023.Based on surgical approach,patients were divided into two groups:the flexible ureteroscopy group(n=40,undergoing ultrasonography-guided ureteroscopic internal incision)and the laparoscopic group(n=35,treated with laparoscopic cyst decortication).Perioperative outcomes(operative time,blood loss,postoperative cyst recurrence)were compared using independent t-test and chi-square test.Results:39 ureteroscopic procedures were successfully completed,with 1 case conversed to laparoscopy,as were all 35 laparo-scopic surgeries were successfully completed.Compared to laparoscopy,the ureteroscopy group demonstrated signifi-cantly reduced blood loss(P<0.05),lower postoperative pain incidence(P<0.05),shorter hospitalization(P<0.05),and decreased operative time(P<0.05).However,ureteroscopy was associated with higher surgical costs(P<0.001)and increased hematuria rates(P=0.045).No intergroup difference in postoperative fever was observed(P>0.05).During 6-12 months' follow-up,all cysts regressed without recurrence in both groups.Conclusion:For par-apelvic cysts,flexible ureteroscopy offers faster recovery and less morbidity than laparoscopy.However,due to ana-tomical complexity and the technique's inherent angular limitations,careful patient selection is warranted.
