Application and training of intraoperative transesophageal echocardiography in the management of renal cell carcinoma with high-grade thrombectomy

Abstract:Objective:To investigate the application and teaching experience of intraoperative transesophageal ultrasonography(TEE)in robot-assisted resection of renal cell carcinoma with high-level tumor thrombus.Methods:TEE data of patients with inferior vena cava tumorthrombusin our hospital from January 2016 to December 2021 were reviewed and analyzed.Theoretical lectures,Heartworks high simulation model operation training and a series of the-oretical combined practical operation tests were conducted for advanced and regular students to help them master the application of TEE in anesthesia management of such surgery.The students working in the Department of Anesthesi-ology and participating in the training on May 10,2023 were randomly selected for the theoretical examination.Re-sults:A total of 106 anaesthesia students were enrolled and trained on the application of TEE in the resection of renal cell carcinoma with high-level tumor thrombus.167 cases of robot-assisted renal cell carcinoma with tumor thrombus were reviewed,including 11 cases of Mayo grade Ⅲ and Ⅳ high-level tumor thrombus.After the training of Heart-Works highly simulated human simulation system,more than 90%of the students can accurately complete the key ul-trasonic section imaging of inferior vena cava thrombectomy,and 85%of the students can accurately identify the ana-tomical structure of the ultrasound subsection.A total of 32 students participated in the theory examination,including 13 males and 19 females,with an average score of 75.5 points,including 67.5 points for objective questions and 8.0 points for subjective questions.Conclusion:After training,the trainees can acquire the basic knowledge of TEE ap-plication in the resection of renal cell carcinoma with high-level tumor thrombus,understand and master the opera-tion training,and greatly improve the young anesthesiologists'grasp of TEE and the application skills of TEE in the resection of renal cell carcinoma with high-level tumor thrombus.
