Abstract:Objective:To investigate the diagnostic value of prostate specific antigen density(PSAD)in combi-nation with the use of the Prostate Imaging Reporting and Data System version 2.1(PI-RADS v2.1)in patients with PSA4-10 μg/L.Methods:90 patients with PSA 4-10 μg/L who underwent transperineal prostate biopsy in our hospi-tal from January 2022 to May 2023 were retrospectively analysed.They were divided into the benign group(n=63)and the prostate cancer group(n=27)according to pathologic findings.All patients underwent multiparametric mag-netic resonance imaging(mpMRI)before the needle puncture,and PSAD and PI-RADS v2.1 scores were calculated.The receiver operating characteristic curve(ROC)assessed the value of PSAD combined with PI-RADS v2.1 for diag-nosis of prostate cancer.Results:All patients finished the transperineal prostate puncture successfully,and 27 cases(30%)prostate cancer werediagnosed by pathology.Compared with the benign group,PSAD was higher and f/t PSA was lower in the prostate cancer group and the differences were statistically significant(P<0.05).There was sig-nificant differences in proportional PI-RADS V2.1 score between the two groups(P<0.001).The optimal cutoff point was PSAD>0.17μg/L·cm3 or PI-RADS V2.1≥4 scores,PI-RADS V2.1 in combinationwith PSAD had a specifici-ty of 64.28%and a sensitivity of 86.68%for diagnosing prostate cancer.Conclusion:PSAD combined with PI-RADS v2.1 has good clinical value for prostate cancer detection in patients with PSA4-10μg/L.
